Tournament Forms

2013 Tournament Team Affidavit form is available for downloading. The WORD form can be electronically filled, printed and then presented to the Credentials Committee. Also available for your use is a PDF form that can be printed and filled out for submission. The Parental Authorization form is also made available! Please print the form out and legibly complete. The form should be submitted at the credentials meeting along with the Tournament Team Affidavit! All information; along with proof of insurance meeting the rule book requirement must be submitted and approved prior to a team being allowed to participate in Dixie Boys Baseball, Inc.
