Tournament Forms

2011Tournament Team Affidavit form is available for downloading. The WORD form can be electronically filled, printed and must be presented to the Credentials Committee. Also available for your use is a PDF form that can be printed and filled out for submission. For your added convenience the Parental Authorization form is made available! Please print the form out and legibly complete. The form should be submitted at the credentials meeting along with the Tournament Team Affidavit! All information; along with proof of insurance meeting the rule book requirement must be submitted and approved prior to a team being allowed to participate in Dixie Boys Baseball, Inc. Note: It is the responsibility of each tournament team (Team Manager & League Officials) to know the rules governing tournament play for Dixie Boys Baseball. Careful attention should be given to all tournament rules & tournament requirements!
